  5. Relationship to Rader (referrals, prior history, connections to staff)
  6. Employment Interests (position desired, salary requirements, availability to work)
  7. Education / US Military Service (High school / other educational histories)
  8. References (3-5 people with emails, not including relatives)
  9. Criminal Background (convictions, pleas, etc are not an automatic disqualification from employment.)
  10. Work Experience (up to 6 entries)
  11. Resume & Cover Letter (optional)
  12. Agreement & Submission
